Ie11 file download underscore

In the past, when I've encountered this issue on a user's device, I've attempted to reset IE without success in resolving the issue. The next time it occurs, I will try it again to confirm. The problem I'm having is that it's not one specific device that is having the issue, it's many users on many virtual machines across our environment. Resetting a user's IE does not seem like it has the potential to fix such a systemic issue.

Do you have any recommendations on how I might "dive deeper" into diagnosing the actual download mechanism on an impacted machine? I'd really like to find out, specifically, why the download is failing or at what point in the process the browser is changing the file extension. Office Office Exchange Server.
